
The Assisted Living Waiver program is offered through the California Department of Health Care Services. This program is designed to assist Medi-Cal beneficiaries to remain in a home-like setting in the community as an alternative to placement in a nursing facility. Those participating in the program receive financial support to help cover the costs of care. Please note that applicants are added to a waitlist.
Enrollment Process
- To be eligible, an individual must already have Medi-Cal benefits in place, with no share-of-cost. The individual must also require a nursing facility level of care.
- Contact a Care Coordination Agency for an initial screening. An updated list of participating agencies can be found here. If eligible, an in-person assessment will be scheduled. Keep in mind that each agency has their own waiting list, and some are shorter than others. It is encouraged to reach out to each agency to determine the size of their waiting list before submitting your application.
- The Care Coordinator will develop a service plan, outlining the services that will be covered by Medi-Cal and those paid by other sources.
- Based on availability, the participant will select a residential care facility from those that accept the Assisted Living Waiver.
Benefits of the Assisted Living Waiver Program
Once enrolled in the Assisted Living Waiver program and residing at an approved facility, an individual is only responsible for the cost of room and board. In 2025, for those receiving $1,599.07 in Supplemental Security Income (SSI), room and board costs are $1,420.07. For those with an income of $1,599.07 or greater, room and board cost $1,440.07. Medi-Cal reimburses the cost of assisted living services such as personal care, laundry, meals, social services, etc.
